-Detailed medical history obtained 

-Discuss goals you may have for your pet

-Meet your pet's care team 

*This fee is credited towards your pet's initial palliative or hospice initial in-person visit if scheduled within 7 days

-Includes full exam by Dr. Kelly Flanagan, DVM, MS, CHPV

-One month of communication and follow-up care

-In-depth review of medical records

-Personalized care plan and email summary

-Prescription plan

-Guided support and hands-on instruction

-Communication and care coordination with the pet's regular and/or referring DVM, if applicable
