A private telehealth consultation with Abby Hull, LVT, VTS (ECC), to help evaluate your pet’s comfort and quality of life. We’ll discuss changes you are noticing, caregiver concerns, and appropriate next steps, which may include continued monitoring, veterinary care, palliative care, or euthanasia planning. This consultation does not include a veterinary examination, diagnosis, prescriptions, or medication changes.

Palliative and hospice patients must live within 30 miles of our Kalamazoo office so our team can provide appropriate in-person follow-up and end-of-life support.

• Discussion of your goals and concerns for your pet
• Introduction to your pet’s care team

 • Comprehensive in-person examination with Dr. Kelly Flanagan, DVM, MS, CHPV
• 30 days of communication and follow-up support
• In-depth review of medical records
• Personalized care plan and written email summary
• Individualized prescription plan, when appropriate
• Guided support and hands-on caregiving instruction
• Communication and care coordination with your pet’s regular or referring veterinarian, when applicable

Every pet’s needs are different. Some patients remain comfortable for months on a stable palliative-care plan, while others benefit from more frequent hospice support. Our team will help you select the level of care that best fits your pet’s condition and your family’s needs. Patients may move between care levels as their needs change.

For pets who are comfortable and doing well on an established palliative care plan.

Recommended every 8-12 weeks for stable patients.

Includes:

• Review of comfort, mobility, appetite, sleep, and quality of life 
• Medication and refill review 
• Discussion of caregiver concerns 
• Veterinary review when needed 
• Recommendations for continued care or an in-person examination

For injections, infusions, subcutaneous fluids, or other scheduled technician treatments. Medication and travel fees are additional.

Recommended when there is a significant change in symptoms, a new concern, or a need to revise the treatment plan.

Travel, medications, injections, and other treatments are billed separately.

For stable patients who are not enrolled in a monthly hospice support plan. Includes review of the medical record and authorization of routine refill requests when appropriate. Medication costs are additional.

Regular rechecks may be required to continue prescribing medications safely.

For pets living with cancer or another serious illness whose symptoms, comfort, or daily function are changing and who need closer monitoring, medication support, and end-of-life guidance.

For pets who are currently stable but benefit from ongoing hospice guidance, monitoring, and support between veterinary visits. 

Includes:

• One scheduled monthly check-in with a technician
• Ongoing quality-of-life monitoring 
• Review of symptoms and response to treatment 
• Routine medication refill management 
• Veterinary review of meaningful changes or concerns 
• Reasonable phone, email, or portal communication 
• Guidance regarding decline and end-of-life decision-making 
• Priority scheduling for in-person appo

For pets with changing or more advanced needs who require closer monitoring and support. 

Includes:

• Two scheduled monthly check-ins with a technician
• Closer quality-of-life and symptom monitoring
• Review of treatment response and changing needs
• Medication adjustment and refill management
• More frequent veterinary review as needed
• Reasonable phone, email, or portal communication
• Guidance regarding decline and end-of-life decision-making
• Priority scheduling for in-person appointments.
