Initial Consultation for End-of-Life Care
-One-on-one conversations with Laura Mellinger, LVT, CCAT, CPEP to establish if Palliative or Hospice care is a good fit for your pet
-Detailed medical history obtained
-Discuss goals you may have for your pet
-Meet your pet's care team
-In-person, phone, or video tele-health options
*This fee is credited towards your pet's initial palliative or hospice initial in-person visit if scheduled within 7 days
Palliative or Hospice Care Initial In-Person Visit
-Includes full exam by Dr. Kelly Flanagan, DVM, MS, CHPV
-One month of communication and follow-up support
-In-depth review of medical records
-Personalized care plan and email summary
-Prescription plan
-Guided support and hands-on instruction
-Communication and care coordination with the pet's regular and/or referring DVM, if applicable
Tier 1 Continued Monthly Care
-Tele-visit, up to 60 minutes with the medical team
